Parent's Information Form
Personal health or medical insurance required for any athlete to participate in athletics at FHSU. Please list this insurance on this form. The top section is information about the student. The next section is information about the parent's or guardians and the insurance that covers the stated student. Please complete the form in its entirety. Please sign the information waiver as this is required by the insurance company for coverage to be in effect. In addition, please send copy of insurance card (front and back).
